 AIDS, amyotrophic lateral sclerosis, Alzheimer’s disease, cancer, chronic traumatic encephalopathy, Crohn’s disease, epilepsy or another seizure disorder, fibromyalgia, glaucoma, hepatitis C, inflammatory bowel disease, multiple sclerosis, pain that is either chronic and severe or intractable, Parkinson’s disease, positive status for HIV, post-traumatic stress disorder, sickle cell anemia, spinal cord disease or injury, Tourette’s syndrome, traumatic brain injury, and ulcerative colitis. 

In addition to having one of the qualifying conditions, conventional therapies and medications should have been tried and demonstrated to fail in order to qualify for the medical marijuana program.

 Qualification is based on your approved medical condition and supporting documents. 


Irrespective of the outcome, the fee is non reimbursable.


 In some cases, additional records are required by the certifying doctor to review prior to signing your certification. If this happens you will be placed in an ON HOLD status pending receipt and review of those records

Preparing for the visit:

For most conditions, the following documents will be required.

1. Ohio State Drivers License

2. A consult note from either your primary care physician or specialist, stating your qualifying condition, duration of the condition, conventional treatments that have been tried and the outcomes.

3. Copies of the most recent investigations including MRI, CT etc depending on the medical  condition.

4.Medical Insurance Card

You can choose any state licensed medical marijuana dispensary to purchase your medicine. You will receive a list from the state. In addition, you can go online and search for one. The dispensary agent at these facilities should be educated about the medical uses of cannabis and be able to guide you through the process. However, at any time, our knowledgeable staff physicians are available for follow up appointments.
